2009 Enrollment Process
1. Enrollment Form - New agents can enroll
by completing our on-line enrollment form. Simply click on
the Enrollment Form link in the left-hand column of this
page.
2. Form W-9 or W-8 - Form W-9 (for U.S.
residents only) or the Form W-8 (for Canadian residents only)
is required to complete the enrollment process. This form is
available to download now by clicking on the links below. You
can also download the forms when you agree to the "Terms and
Conditions" prior to submitting your enrollment form.
3. Submit Form W-9 or W-8 - Please mail
your completed original signed Form to Aston Hotels &
Resorts - STARs Online, 2155 Kalakaua Ave, 5th Flr,
Honolulu, HI 96815-2398. NO XEROX, FAXED OR E-MAILED COPIES
WILL BE ACCEPTED FOR ENROLLMENT.
4. User ID and Password Issued - Once the
Form W-9/W-8 has been submitted, Aston Hotels &
Resorts will send an e-mail to the agent with his/her User
name and Password within 1 - 2 business days.
